Qualified Mental Health Professional Community Services (QMHP-CS) for Assertive Community Treatment (ACT) Team

Role Responsibilities

The ACT Team QMHP-CS will serve as an integral member of the Assertive Community Treatment Team (ACT Team) for the Behavioral Healthcare division. The position requires participation on a multi-disciplinary team to provide services consistent with the ACT model and works in collaboration with other professionals involved in an individual’s care (i.e. Psychiatrists, Nurses, Employment Specialists, LCDC, etc.), as well as with individuals, their family members and other collaterals, and community partners to promote each individual’s recovery. The position works with the most difficult to engage individuals, including those with criminal histories who have recently been incarcerated. The ACT Team is a key component of a collaborative aimed at reducing arrests and incarceration of individuals with mental illness in Fort Bend County. The ACT Team QMHP-CS conducts Uniform Assessments (ANSA), develops and reviews recovery plans, uses evidence based practices (i.e. Illness Management and Recovery, Motivational Interviewing, Person Centered Recovery Planning), and completes accurate, timely documentation of all services provided. The position requires participation in daily team meetings and provision of 80 billable hours of service to individuals each month, 80% of which are in the conducted in the community. The position reports directly to the ACT Team Leader and will perform other duties as assigned by the ACT Team Leader or Crisis Services Manager. Routine hours are 8:00am to 5:00pm, Monday through Friday, with flexibility required to meet program needs outside of routine hours. The position includes rotation of after-hours, on-call crisis coverage, primarily via phone and in coordination with MCOT. Must have reliable vehicle for travel throughout Texana catchment area, with mileage reimbursed. Responsible and accountable for adherence to and promotion of the compliance program.

Salary

  • 90% $20.88 hour to start;
  • 95% $22.04 hour at 6 mos. with satisfactory performance
  • 100% $23.20 hour at 1 year with satisfactory performance

Required Experience

  • 1 year of experience working with adults with serious mental illness

Preferred Experience

  • Prior experience in a community mental health center
  • Spanish speaking

Skills, Education and/or Certifications Required

  • Bachelor’s degree in psychology, social work, or human services related field
  • Texas driver’s license in good standing as defined by Texana Center’s procedure

Job Related Physical Requirements

  • Must be able to perform SAMA and CPR.
  • Must be able to perform moderate lifting and carrying (15-44#), pushing, stooping, repeated bending, reaching above shoulder, simple grasping, hearing, walking, sitting and twisting.
  • Must have ability to see, write, operate office equipment, operate a motor vehicle and have depth perception.
  • Qualified individual must be able to perform these functions with or without reasonable accommodations.
