Job Description Job Description POSITION SUMMARY: The Case
Management Assistant (CM-A) plays a pivotal role in supporting Case
Managers by facilitating healthcare service delivery and discharge
planning in collaboration with the multidisciplinary team. This
position involves assisting in the coordination of patient care,
ensuring timely access to necessary resources, and managing
documentation to ensure all stakeholders are well-informed.
JOB-SPECIFIC RESPONSIBILITIES: • Service o Consistently supports

policies on Health Insurance Portability and Accountability Act
(HIPAA), designed to prevent or detect unauthorized disclosure of
Protected Health Information (PHI) o Collaborates effectively with
colleagues and other departments to ensure seamless service
delivery. • Excellence: o Coordination of Service • Schedule and
coordinate patient appointments, transportation, and other
necessary services. • Liaise with healthcare providers, community
organizations, and service agencies to ensure comprehensive patient
care. • Assist Case Managers with preparations for the treatment
planning conferences. • Assist the Case Managers with obtaininging
discharge planning resources and follow-up for placements status •
Coordinates the ordering of Durable Medical Equipment (DME) and
supplies • Maintain campus-wide recertification list o
Documentation and Record-Keeping: • Maintain accurate and
up-to-date patient records, ensuring compliance with
confidentiality and privacy policies. • Prepare and distribute case
management plans, summaries, lab reports, and therapy notes to
relevant stakeholders. • Create transport packets to accompany
medical appointments: history & physicals, lab orders,
prescriptions, and other physician orders as necessary. • Review
medical bills for accuracy, forward to appropriate stakeholders or
guardians for payment o Administrative Support: • Perform clerical
duties such as data entry, filing, and managing correspondence. •
Mantains Case Management files • Assist in preparing for case
reviews, audits, and team meetings. • Patient Experience and
Advocacy: o Act as a liaison between patients, families, and the
healthcare team, providing information and support as needed. o
Assist patients and families in completing necessary paperwork and
understanding available resources. o Complete a six-month follow-up
call to all patients/guardians for a status update. o Complete
discharge satisfaction surveys • Quality Assurance and Compliance:
o Ensure all activities adhere to healthcare regulations and
organizational policies. o Participate in quality improvement
initiatives to enhance service delivery. o Promotes of a culture of
patient safety which results in the identification and reduction of
unsafe practices. • Professional Growth and Continuing Education o
Completes annual education requirements. o Maintains competency, as
evidenced by completion of competency validation requirements. o
Maintains competency and knowledge of current standards of
practice, trends, and developments. o Participates in relevant
workshops, seminars, and continuing education courses to stay
current with industry trends, healthcare regulations, and best
practices. • Finance: o Promotes stewardship of hospital resources
while ensuring quality patient care. • Performs other duties as
assigned. POSITION QUALIFICATIONS: EDUCATION: • A high school
degree required EXPERIENCE: • Two (2) years of case management
assistant experience preferred • 3-5 years healthcare experience
preferred SKILLS: • Strong interpersonal and communication skills •
Proficiency in MS Office software • Excellent organizational and
time-management skills • Ability to gather resources, referrals, or
case-specific information when necessary. LICENSURE/CERTIFICATION:
